We are conveniently located near the intersection of Fitzhugh and Central Expressway, on the Southeast corner. There are 6 reserved spaces on the right as you enter the parking lot. Covered parking for the building is provided free of charge.

If you are having an urgent medical problem, please do not email us as emails are not checked after hours, and there may be a delay getting back to you. Instead, for active/urgent medical problems, please call our office directly at (214)303-1033.

If you are interested in becoming a new patient, please download the new patient forms from the "Patient Resources" tab and either fax, email, mail, or hand deliver them along with a copy of your drivers license and insurance card. You will then be contacted to set up an appointment time.

Please contact us either by phone or log into your patient portal. You can do this by going to http://upg.myupdox.com. From here you can confirm your appointment, make new appointments, send us messages (not for acute care issues) and view your lab results. If you have not yet logged in, your user name is your email address. You will then verify your identity and create a password.
